PASS provides eligible individuals an easy,
affordable solution to their costly prescription medications
by offering a low-cost, worry-free, full-service prescription
assistance program designed to utilize pharmaceutical
company Patient Assistance Programs that make medications
available to eligible individuals free of charge.
How PASS Is Your Advocate
To participate in a Patient Assistance Program, each pharmaceutical company requires a lengthy
qualification, application and approval process for each medication you take. PASS does all
the legwork for you to provide a worry-free, trouble-free way to
apply for assistance with your prescription medications. Here's how:

1. To enroll in PASS just complete the enrollment form and return it to PASS. If you have any questions at any time during the process, just call our friendly assistants who are happy to help you. This enrollment makes you a lifetime member—so you never have to enroll again.
You can download the form or contact PASS and we will mail you one.

Once PASS has received and reviewed your enrollment
form, we determine your eligibility,
based on your personal information and medication(s),
and then notify you, via mail, as to what
medication(s) you are eligible for assistance.
In addition to our letter, you will receive
a request for any other documents required
by the Patient Assistance Program(s) (such
as verification of income, etc…), as
well as authorization forms, so that PASS
may make application on your behalf to the
applicable programs. |

PASS Does the Rest!
1. We complete
the applications on your behalf and send them
to your prescribing physician(s) for their
signature and prescribing information.
2. Once the completed applications are received
back from your physician, we submit them to
the appropriate pharmaceutical company sponsored
Patient Assistance Program.
3. We follow-up with each program to confirm
the status, and notify you of the result of
each submitted application. |

PASS takes care of your ongoing prescription assistance needs.
- Handles refills, when
required, or new prescriptions for approved
medication(s).
- Reapplies to each Patient Assistance
Program, as required, to continue your
ongoing assistance of receiving your approved
medication(s).
- Prepares a new application if and when
there is a change or addition to your
medication regimen.
